It is important your physician and family are aware of your wishes in the event you ever lose your ability to make decisions for yourself. It is advised, and often required, that your advance planning directives be in writing.
Examples of advanced planning documents you might want to consider include:
- A HEALTHCARE POWER OF ATTORNEY (POA) provides for you to name a person(s) authorized to make decisions about your health care if you become unable to decide for yourself.
- A LIVING WILL informs your family of your health care decisions should you become unable to do so.
When you are admitted to Caldwell UNC Health Care you may be asked to provide a copy of any advance planning document you might have.
When visiting your Caldwell Physician Network care provider office/clinic you may want to discuss these advance planning options.
