Southfork Medical Park's family care physicians provide comprehensive healthcare, diagnostics and treatments for well and sick visits for every member of your family.

We would like to welcome you to our Patient Centered Medical Home (PCMH). A Patient Centered Medical Home is a team-based approach to health care.  The team is made of health care providers, support staff, and most importantly – YOU.  As your medical home, we will take care of you when you are sick and when you are well.  We will help you set and achieve health-related goals.  We will work with you, with additional health care providers, and with other resources in our community to coordinate your care.

New Patient Information

If you would like to become a patient at Southfork Medical Park, please call our office to schedule a new patient appointment with one of our providers. You will be asked to submit the New Patient Packet below.

Please be sure to bring your completed packet back to our office at least three days prior to your appointment. If you need assistance completing the packet, please contact our office. If for any reason you have not completed and returned this packet at that time, our office will contact you to have it brought in by the end of that business day. Failure to do so will result in your appointment being canceled and rescheduled for a later date.

As a courtesy, two days prior to your appointment, our office will send out a reminder call regarding your appointment date and time. Thank you for considering us to be your primary healthcare provider!
