Robbins Medical Park is home to adult and internal medicine as well as the service of PLUS Urgent Care. Robbins Medical Park offers specialized healthcare, diagnostics and treatments for well and sick visits for the adult members of your family and urgent care for every member of your family.

New Patient Information

If you would like to become a patient at Robbins Medical Park, please call our office to schedule a new patient appointment with one of our providers. You will be asked to submit the New Patient Packet below.

Please be sure to bring your completed packet back to our office at least three days prior to your appointment. If you need assistance completing the packet, please contact our office. If for any reason you have not completed and returned this packet at that time, our office will contact you to have it brought in by the end of that business day. Failure to do so will result in your appointment being canceled and rescheduled for a later date.

As a courtesy, two days prior to your appointment, our office will send out a reminder call regarding your appointment date and time. Thank you for considering us to be your primary healthcare provider!
