Westpointe Medical Practice's family care providers offer comprehensive healthcare, diagnostics and treatments for well and sick visits for every member of your family.

We would like to welcome you to our Patient Centered Medical Home (PCMH). A Patient Centered Medical Home is a team-based approach to health care.  The team is made of health care providers, support staff, and most importantly – YOU.  As your medical home, we will take care of you when you are sick and when you are well.  We will help you set and achieve health-related goals.  We will work with you, with additional health care providers, and with other resources in our community to coordinate your care.

Meet Our Team

Our Patient Centered Medical Home specializes in providing excellent health care for patients of all ages. We work closely with each patient to provide care for chronic and acute medical conditions and help you set and achieve health-related goals, emphasizing preventive health screenings and education. Our team provides comprehensive health care and offers a wide range of convenient services for our patients, both onsite and through the advanced medical technologies available through Caldwell Memorial Hospital. In addition, we have access to a wide variety of specialists and diagnostic testing facilities should your care necessitate a referral of any kind.

To be an active participant in your care, you should:
• See your provider at regularly scheduled intervals or as needed.
• Maintain and update your medical history with your provider.
• Tell your provider about any other health care professionals who care for you.
• Remain engaged in your care plan and ask questions about your treatment plans. Give feedback about the care you are receiving.
• Feel empowered to take care of your health and collaborate with your provider to make decisions about your treatment.

Important Patient Information about Your Medical Home

Clinical Advice & Prescription Refills

During office hours, contact the office and follow the prompts to speak with the clinical staff or request a prescription refill. For non-urgent clinical advice or prescription refills, you may also send a request through My UNC Chart. Patients have access to extended hours care through Caldwell Memorial Hospital’s PLUS Urgent Care services and after-hours care by contacting Caldwell Memorial Hospital and asking for the provider on-call.

To Schedule/Cancel an Appointment

Appointments can be scheduled/cancelled by calling the office through My UNC Chart. More information on My UNC Chart is included in the packet. Note: Please call the office in advance if there is a change or cancellation. A cancellation fee will be applied if you are unable to contact us within 24 hours.

Before Your Appointment

•Write down any questions that you may have.
•Put your questions in order of importance so that you are sure to cover the most important question first.
•Bring a list of other health care professionals that have taken care of you since your last visit---include any specialists, hospitalizations or ER visits.
•Write down the medications that you are taking, or bring all of the bottles with you to the appointment, including over-the-counter and herbal remedies.

Normal & Extended Hours

Our regular business hours are Monday-Friday, 8:00 a.m.- 5:00 p.m. Additionally, our patients have access to extended hours care through Caldwell Memorial Hospital’s PLUS Urgent Care locations in Lenoir, Hudson, Granite Falls and Blowing Rock from 8:00 p.m.–7:30 p.m. Monday through Friday and from 10:00 a.m. to 6:00 p.m. Saturday and Sunday. No appointment is necessary to been seen for PLUS Urgent Care.

Transferring Records

Our Patient Centered Medical Home is most effective when we have a complete medical history for our patients and information about care obtained outside the practice. To transfer your records to our practice, please fill out the Release of Information (ROI) form (enclosed). For additional forms or assistance, please contact our office.

Behavioral Health Services

Through referrals, patients have access to community-based Behavioral Health Services and Medication Consultation. Referrals are made to Behavioral Health Service providers for direct contact.

What is a Family Physician, Nurse Practitioner or Physician Assistant?

A family physician is a doctor who is devoted to comprehensive health care for people of all ages. Family medicine’s cornerstone is ongoing, personal, patient-physician relationship focusing on integrated care. A nurse practitioner or physician assistant is an advanced practice medical professional that blends clinical expertise in diagnosing and treating health conditions with an added emphasis on disease prevention and health management. Much like a family physician, a nurse practitioner or physician assistant works with patients throughout their lives, diagnosing illness, conducting exams and prescribing medication.

What if I Need to See the Doctor Right Away?

For life-threatening emergencies, please call 911 or go to the nearest Hospital Emergency Room for treatment. For non-emergency situations, call our office at (828) 757-8950. You may also access Caldwell Memorial Hospital’s PLUS Urgent Care service at one of our locations in Blowing Rock, Granite Falls, Hudson or Lenoir.

What if I Don’t Have Insurance?

We are committed to serving all patients regardless of financial or insurance status. If necessary, we will work with you to make financial arrangements and/or provide information on how to obtain coverage. However, please be aware that you will be required to pay for your initial office visit upon check-in. Please contact our office staff if you have any questions.
