Diabetes Education & Support

The Center for Diabetes Health has been recognized by the American Diabetes Association since 2000 for providing quality diabetes education services to our patients.  The program offers Diabetes Self Management Training for people who have been diagnosed with Type 1 Diabetes, Type 2 Diabetes, and Gestational Diabetes, as well as people who are trying to reduce their risk of diabetes.

Upon entry into the program, you will meet with a diabetes educator who will help you develop a plan which meets your diabetes education needs.  While you are participating in the diabetes education program at the Center, our diabetes educators will work closely with you and your physician to help you learn how to achieve better diabetes control.

Additional services offered at The Center for Diabetes Health include:

  • Insulin pump training,
  • Glucose sensor training,
  • Free seminars regarding diabetes-related topics,
  • "Healthy4Life", a program designed to assist children and their families to adopt healthier nutrition and activity habits.

All services are physician-referred.  Please ask your physician to complete and submit the referral form below.
